Dear Parent/Guardian,

If you would like your child to be seen by the Smile Safari team when we visit your child’s daycare, please fill in the Dental Consent form below.

We ask that the Dental Consent form be filled in and returned to us no later than one week prior to our visit so that we can check your child’s CDBS eligibility.

Smile Safari will provide your child a dental check up and clean on the day consisting the following services (where clinically relevant):

• Comprehensive oral examination
• A dental clean to remove plaque and tartar
• A remineralising agent to help strengthen the teeth
• After the appointment all children will receive at dental gift pack. A personalized report and invoice will also be emailed to you.

For all children eligible under Medicare’s Child Dental Benefits Schedule (CDBS) we will bulk bill Medicare so there is no out of pocket cost to you.

If you would like us to check your child’s eligibility please fill in the form below and we can check on your behalf.

If you child is not eligible through Medicare’s CDBS or you have used up your remaining benefit we will contact you to let you know. Alternatively your child can still receive the same services at a fixed cost of $75 payable in full before our visit. If you have private health cover that includes dental we will issue you an invoice after your child’s treatment to make a claim.
